The IJP Parish Registration / Census Form is available in two versions:

Fill in MS Word Form – fill in answers on your computer (Microsoft Word Form) - convenient for emailing back to IJP or

Print out PDF Form – print form to write in answers

1. Fill in MS Word Form Directions

  Click this link: Census Registration Word Form to download

  Click “Save” (note where form will be saved) Click Save (again), Click “Open”

  Fill in the form in the shaded areas: use tab key to move through fields, use mouse for check boxes.

  After completing form click “Save” “Close”  

Send Completed Census Form to IJP:

  Email form as an attachment to: census@ijpparish.org  You will get a reply confirmation email back from census@ijpparish.org that we received  your census.  If you do not get a confirmation reply email, please try to send with Internet Explorer.

  Fax to: 708-799-5462

  Mail printed form to:  IJP Parish, 1131 Douglas Avenue, Flossmoor, IL 60422

  Drop printed form in Offertory Basket.

2. Print out PDF Form Directions

Click this link: Census Registration PDF Form.

After the form opens, click the “Print Icon” on the toolbar to print the form.

Write in your information, then send the completed form back to IJP.

  Mail printed form to:  IJP Parish, 1131 Douglas Avenue, Flossmoor, IL 60422

  Email form as attachment to: census@ijpparish.org  

    (copy and paste address in your email "compose to" box)

  Drop form in Offertory Basket.
